Case summary

In 2013, a female child H (aged 8) died whilst in the care of her mother. We will review the details of the case, including:
Father’s ongoing mental health issues
Mother’s allegations of domestic violence
A distressed sibling witnessing the family breakdown and its fallout

We will also discuss the recommendations of the Serious Case Review, including the actions of a parent support advisor who acted outside of the boundaries of her role, with little support and guidance.
